The thoracoscopic lobectomy has been recently introduced as a new method for the surgical treatment of non-small cell lung cancer. The aim of this study was to compare our results with those of a previous series of patients who underwent lobectomy by open thoracotomy. After approval from the institutional review board, we analyzed a consecutive cohort of patients in whom video-assisted thoracic surgery lobectomy was performed for the treatment of stage I non-small cell lung cancer. Surgeon A performed the operations in 16 patients and surgeon B performed the operations in 17 patients, for a total of 33 consecutive lobectomies. We compared our results with those of a control cohort of 37 consecutive lobectomies performed by surgeon A during the same period before the introduction of the thoracoscopic technique. The operative mortality rate was 0% in both groups (P = 0.40). The chest tube output was significantly less after thoracoscopic lobectomy than after open lobectomy (218 versus 332 mL, P = 0.012). The duration of chest tube drainage was significantly less after thoracoscopic than after open lobectomy (4 versus 6 days
